Terms of Service Policy
Effective 3rd April 2026

Please choose carefully as I am are unable to refund booking/bookings due to change of mind. In the event of illness, a minimum of 6 hours notice is required for 100% of the booking to be credited. In the event you provide less than 6 hours notice of illness, 40% of the booking will be credited to your Setmore account. Credits are valid for 2 years from date of issue. Credits cannot be exchanged for cash. Please ensure you complete this form – Jazz’s Babysitting Client Intake Form honestly and to the best of your knowledge. Please ensure you read and sign my Qualifications, Clearances and Certificates Document prior to your booking. Please ensure the 2 Emergency Contacts listed in this form – Jazz’s Babysitting Client Intake Form are available and are in Perth on the date of your booking/bookings. If your child requires any medication to be administered during the booking (pharmaceutical or homeopathic), please ensure you complete Part A of the Nannysure Permission Slip to Administer Medication Form and provide Jazz with the correct medication listed in the Nannysure Permission Slip to Administer Medication Form at the start of the booking/bookings. Please ensure food is prepared prior to my arrival. For takeaway/food deliveries, please ensure you order/pay prior to my arrival or leave the correct cash at the start of your booking as I am unable to pay for food deliveries. During the booking, the children and I cannot leave the boundary of the property (go on any outings). I strictly do not transport children in the car. Swimming pools and spas are strictly off limits. I am unable to clean your home or cook meals outside of caring for your children.                                                                                                           

If you are going to be late home, please contact me via SMS as soon as possible. The latest finish time for babysitting is 12am (excluding New Years Eve/Events which has a latest finish time of 1am). If you arrive home later than the extended agreed overtime, an additional charge of $80 will apply ontop of the applicable hourly rate. If you are not contactable when the agreed extended overtime has lapsed, your emergency contacts will be called to relieve me. Overtime is expected to be paid to me via EFTPOS before I leave. 

I will endeavour to follow your requests outlined in this form – Jazz’s Babysitting Client Intake Form, however, if your requests do not follow best practice, I will always fall back on my training as an educator and what I am most comfortable with. 

For my health and safety I am unable to care for children who are sick or on a course of antibiotics. If I arrive to sick child/children, the booking will be terminated immediately and no further care will be provided by Jazz’s Babysitting. I have a zero tolerance policy for abusive/disrespectful behaviour from parents, guardians, guests and children. In the event abusive/disrespectful behaviour occurs, the booking will be terminated immediately, a Police report will be made and no further care will be provided by Jazz’s Babysitting. In the event you do not come home, your emergency contacts are not-contactable or Overtime is not paid, the Police will be called and no further care will be provided by Jazz’s Babysitting.
